What to include in your notice

  1. Your physical or electronic signature, or that of the person authorized to act for the rights holder.
  2. Identification of the copyrighted work you say has been infringed.
  3. The URL of each page containing the material you want removed.
  4. Your contact details: name, postal address, phone number, and email address.
  5. A statement that you have a good-faith belief the use is not authorized by the rights holder, its agent, or the law.
  6. A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act for the rights holder.

Counter-notices

If you believe material was removed in error, you may send a counter-notice to the same address with your signature and contact details, identification of the removed material and where it appeared, a statement under penalty of perjury that you have a good-faith belief it was removed by mistake or misidentification, and your consent to the jurisdiction of the appropriate court.
